Tax Preparation Checklist

We aim to make taxes as easy as possible for you! We’ve created this list of documents and things you need to bring when you come to meet with us. We’ll go over this list with you during our consultation meeting and work to get you the best refund possible. 

Personal Information

  • Social security or tax ID number
  • Social security or tax ID number of your spouse, if applicable
  • Dates of birth of all dependents 
  • Social security or tax ID numbers of all dependents
  • Driver’s licenses for taxpayer & spouse
  • Driver’s licenses for any children who have one and are filing a return
  • Last year’s tax return
  • Spouse’s tax return from the previous year, if filing jointly
  • If first year customer, please bring actual birth certificates and social security cards for all family members

Income Information

  • W-2 forms from all employers you worked for in the past year
  • If you are filing a joint return, W-2 forms from all of your spouse’s employers in the last year
  • Information regarding investment income, including proceeds from the sale of bonds or stocks, income from foreign investments, interest income, and/or dividend income
  • Income from local and state tax refund from last year
  • Business income and accounting records from businesses you or your spouse own
  • Unemployment income
  • Rental property income and expenses 
  • Social security benefits
  • Proof of miscellaneous income, such as lottery winnings, gambling winnings, etc.

Income Adjustments, if applicable

  • Homebuyers tax credit
  • Green energy credits
  • IRA contributions, traditional and roth
  • Mortgage interest
  • Student loan interest
  • Contributions and distributions to health savings account (HSA)
  • Self-employed health insurance
  • Moving expenses, for military moves only
  • Education costs, 1098-T from the University and a print out of the student financial account showing the payments made
  • Qualified medical expenses
  • Long-term care insurance premiums paid
  • Real estate taxes paid on your principle residence

Dependent Care, if applicable

  • Education costs
  • Contributions to Indiana Choice 529 College Savings Account
  • Childcare costs, including preschool costs
  • Adoption costs

Charitable Contributions, if applicable

  • Charitable donation receipts
  • Donations to Indiana colleges, even if they do not count to itemize

Bank Information

  • Bank account number
  • Bank routing number
